The Woodville Tdap clinic date will be ​Friday, May 7th, 2021,​ from 9:00 a.m.-12:00 p.m.

This is a required shot for all incoming 7th grade students.



You will find the following forms attached to this email (from the Department of Health):



  *   Tdap Flyer

  *   CDC Tdap Vaccine Information Sheet (VIS)

  *   Tdap Initiation of Services Form

  *   Tdap Vaccine Consent Form

The flyer states consent forms may be e-mailed to [log in to unmask]<mailto:[log in to unmask]> or given to the clinic aide prior to May 7th.



You may also take your student to his/her pediatrician to obtain the TDAP shot prior to the start of 7th grade.
