The Woodville Tdap clinic date will be Friday, May 7th, 2021, from 9:00 a.m.-12:00 p.m.

This is a required shot for all incoming 7^th grade students.

You will find the following forms attached to this email (from the Department of Health):

• Tdap Flyer
• CDC Tdap Vaccine Information Sheet (VIS)
• Tdap Initiation of Services Form
• Tdap Vaccine Consent Form

The flyer states consent forms may be e-mailed to [log in to unmask] or given to the clinic aide prior to May 7^th.

You may also take your student to his/her pediatrician to obtain the TDAP shot prior to the start of 7^th grade.
