Dear Parents/Guardians -



The School Health Division of the Leon County Health Department is preparing for the annual Tdap Clinics. The Tdap Clinics will allow 6th grade students not in compliance with the Tdap immunization to be vaccinated at SCMS at the time noted below.



Please see the attached flyers and consent form for more information. Students may return their consent forms to the SCMS clinic or the [log in to unmask]<mailto:[log in to unmask]> inbox by the February 24th deadline.



Only students that have turned in their Tdap Vaccine Consent form and Initiation of Services Consent form will be allowed to receive the shot.  The Tdap immunization will be done between the hours of 9:00 a.m. - 12:00 p.m. on Tuesday, March 1st at SCMS in the Rizzo Room.
