The Florida Department of Health requires all 7th Graders to have the Tdap (1 dose) Tetanus-Diptheria-Pertussis vaccine.  Documentation of the Tdap is required BEFORE your student starts 7th Grade. Please submit the Florida Certification of Immunizations (Form DH 680) to our registrar, Ms. Dorsey, [log in to unmask]<mailto:[log in to unmask]>   Immunizations are available from your private medical doctor or the Department of Health (see free opportunity below).

Fairview will have a TDAP Clinic on our campus on Thursday March 28th. The attached consent forms are due Tuesday, March 21st. The TDAP Clinic will be held on campus  from 9:00 a.m.-12:00
