For those students who will be attending the SeaWorld field trip-

Yesterday, you should have received a very important packet of information to complete.  Please return these completed forms by Tuesday, April 12.  These completed packets may be given either to Ms. Osborne in room 28 or to Ms. Mosley in Guidance.

PLEASE NOTE-Ms. Osborne will be attending a math competition on Friday, April 8, and will NOT be on campus.  Any packets that are returned on that day should ONLY be turned in to Ms. Mosley.

A couple of questions that you may have about these forms:
*       Consent for Medical Treatment-you do NOT need to put an ID#.  We will take care of that for you.
*       Assumption of Risk and Indemnity Agreement-the third blank reads: "...assert that I have had an opportunity, prior to enrolling my child/ward in the Camp, to review these materials which include but are not limited to:____________...",  Insert the words "camp materials, sample itinerary" in that blank.
