The Cobb Middle School Tdap Clinic will be held on Wednesday, May 26 from 9:00 AM to 12:00 PM. The Tdap vaccine is a required immunization for 7^th graders.

Please complete each of the attached forms and either email them to [log in to unmask] or turn them into the front desk no later than Tuesday, May 25:
