Dear Parents/Guardians of rising 7th Grade Students:

Per state statute, all student that enter 7th grade for the 2021-2022 school year must have the following immunization:

   __X__    One dose of tetanus-diphtheria-pertussis (Tdap) vaccine                                                                                                                   (This immunization is typically given to students at 11-12 years of age)

Please have you child immunized as soon as possible for next school year.  School immunizations may be received from your child’s private doctor or at the Florida Department of Health - Leon, with payment based on a sliding scale.

To schedule an immunization appointment, or for more information, please call the Leon County Health Department Immunization Program Office at 850-404-6403.

All children receiving immunizations through the Leon County Health Department must be accompanied by a parent or legal guardian.  When taking your child for immunizations, please bring this letter with you, along with a copy of your child’s complete immunization record.

Once your child has received the immunization to enter 7th grade, please bring an updated copy of your child’s immunization record, on a Florida Certification of Immunization Form, to our school registrar by August 1, 2021.  Without proof of immunization, students will not be permitted to attend school and will not receive a schedule for the 21-22. Please submit immunization records to Ms. Harris, our registrar, in the guidance office. If you have any questions, please do not hesitate to call 850-617-5353.
