Dear Parents/Guardians of Fifth Grade Students,
Leon County School Board policy 3.02 “Assignment of Students” requires verification of the current residences of all students transitioning from elementary school to middle
school and from middle school to high school before the student can be admitted to the receiving school.
Therefore, it is necessary for the parents/guardians of each student currently enrolled in 5th grade at Apalachee Tapestry Magnet School of the Arts to provide
proof of residence in order to complete their child’s transition to middle school, if the student are promoted at the end of his/her fifth grade school year.
Two current proofs of residence are required. Proof of the primary residence must include
one of the following documents: homestead exemption, mortgage deed, a lease/rental agreement, or property tax record;
and at least one other form of appropriate documentation such as a driver’s license, voter registration card, utility bill or insurance bill. Photocopies of the required documentation will suffice.
The two required proofs of residence, submitted in any of the following ways to the attention of Mrs. Sellars, Registrar. Fax to (850) 922-0202, emailed to
[log in to unmask] and/or mailed via U.S Postal Service to Apalachee
Tapestry Magnet School of the Arts; or in person to the main office by you or your child.
Please include your child’s name and date of birth on the documents as well as a telephone number where you can be reach and/or an email address. If this information is
not on the documents, it will be incomplete.
This information must be submitted to the school by
Monday, April 12, 2021 to ensure your child smooth transition to middle school. Report cards will be issued only to students who have had their addresses verified through this process.
