If your child is in need of a physical for sports this year, the Bond-Housing Authority Primary Care Center is available to assist you.  Bond-Housing Authority Primary Care is located at 1704 Joe Louis Street. If your child is a 6^th grader, please keep in mind that he/she will need to have immunizations prior to their 7^th grade year.  The Bond-Housing Authority Primary Care can assist you with getting those immunizations now.  Please contact them at 850-391-4799 to set up your appointment.
