LEON COUNTY HEALTH DEPARTMENT
PROVIDES SPECIAL BACK-TO-SCHOOL IMMUNIZATION CLINICS

Students who will be entering kindergarten or seventh grade next school
year must have certain immunization documentation on file at school before
the start of the new school year. It is not too early for children to be
immunized for the 2006-2007 school year!

The Leon County Health Department continues to provide special immunization
clinics targeting students who need immunizations for school attendance.
Infants and young children needing immunizations may also be immunized at
these special clinics. While an appointment is encouraged, children may be
seen on a walk-in basis during the free immunization clinics listed below:

Thursday, July 13, 2006 4:00 p.m. – 6:30 p.m. Roberts & Stevens
Clinic

      1515 Old Bainbridge Road
Wednesday, July 19, 2006 4:00 p.m. – 6:30 p.m. Leon County
Health Dept.

        2965 Municipal Way
Thursday, July 20, 2006 4:30 p.m. – 7:00 p.m. Dr. B. L. Perry
Branch Library

       2817 South Adams Street

All children receiving immunizations through the Health Department must be
accompanied by a parent or legal guardian. Parents should bring a copy of
their child’s complete immunization record.

Any child receiving immunizations through the Leon County Health Department
on or before
July 31, 2006, may register for a drawing for one of two $25.00 Governor’s
Square gift certificates donated by VISTA Healthplan.

To schedule an immunization appointment, or for further information, please
call the Leon County Health Department at 488-2223.
