Students who are entering kindergarten or seventh grade this school year must have certain
immunization documentation on file at school before the start of the school year.

The Leon County Health Department is providing a special immunization clinic at the Northeast
Branch Library, 5513 Thomasville Road, on Thursday, August 3rd, 2006, from 4:00pm to 6:30pm.

All children must be accompanied by a parent or guardian, and a copy of the child's complete
immunization record must be provided.

Children will be seen on a space-available, walk-in basis, and all immunizations are free.
