Incentive to Receive Back-To-School Shots
at the Leon County Health Department During July 2005
All children who receive childhood immunizations at the Leon County Health
Department by Friday, July 29, 2005, may register for a chance to win a
$25.00 gift certificate to select stores in our community. Four gift
certificates were donated to the Health Department to encourage early
completion of childhood immunizations, including those immunizations
required for kindergarten** or seventh grade entry.
During the month of July, the Leon County Health Department will provide
free childhood immunization clinics at various sites in our community. Each
Tuesday evening from 4:30 p.m. – 7:00 p.m., childhood immunizations are
available at the Roberts & Stevens Clinic, 1515 Old Bainbridge Road. To
schedule an immunization appointment for this clinic, please call 488-2223.
Free immunization clinics will also be held at the county library locations
listed below. No appointment is needed for these special childhood
immunization clinics.

Parent or legal guardian must accompany all children receiving
immunizations at the Health Department. Please bring a copy of your
child’s immunization record, and, if available, social security card.
**IMPORTANT! During the special clinics listed above, varicella
(chickenpox) vaccine will only be available at the Roberts & Stevens Clinic.
